We may share your information under the following circumstances:

• With our fitness center partner, when necessary to support your wellness and fitness plan (with your consent, as required by law).
• With service providers that help operate our business (e.g., website hosting, payment processing, email communications).
• As required by law, including in response to legal requests or to comply with health regulations (HIPAA, if applicable).
• In the event of a business transfer, such as a merger or acquisition.

We do not sell your personal information.

HIPAA Compliance & Protected Health Information (PHI)

As a medical and wellness provider, we are required by the Health Insurance Portability and Accountability Act of 1996 (HIPAA) to maintain the privacy and security of your Protected Health Information (PHI). PHI includes any information related to your health status, treatment, or payment that can identify you.

We will only use and disclose your PHI:

• For purposes of treatment, payment, and healthcare operations
• With your explicit written consent, where required
• When permitted or required by law or regulation
• To fulfill our obligations with public health authorities or oversight agencies
• In limited situations for research, legal proceedings, or law enforcement, in accordance with HIPAA guidelines

We implement strict administrative, physical, and technical safeguards to protect your PHI. You have the right to:

• Access and obtain copies of your health records
• Request corrections to your health records
• Receive an accounting of certain disclosures of your PHI
• File a complaint if you believe your privacy rights have been violated
